Acute Necrostising Pancreatitics

Acute Necrotising Pancreatitis

Advanced Critical & Surgical Care for Life-Threatening Pancreatic Inflammation

Overview

Acute Necrotising Pancreatitis (ANP) is a severe, life-threatening form of pancreatitis in which parts of the pancreas and surrounding tissues die (necrotize) due to inflammation and lack of blood supply. This condition often requires intensive care, multi-disciplinary management, and sometimes surgical or minimally invasive intervention to remove dead tissue and control infection.

Causes

  • Gallstones blocking the bile duct
  • Excessive alcohol consumption
  • High triglyceride levels
  • Certain medications or infections
  • Abdominal trauma or post-surgical complications
  • Autoimmune or idiopathic (unknown causes)

Symptoms

  • Sudden, intense upper abdominal pain (often radiating to the back)
  • Persistent vomiting and nausea
  • Abdominal bloating or tenderness
  • Fever, chills (indicating infection)
  • Rapid heartbeat and low blood pressure
  • Jaundice (yellowing of the eyes/skin)
  • Signs of systemic shock or organ failure in advanced cases

Diagnosis

Diagnosis is made through a combination of clinical evaluation, imaging, and laboratory tests.

  • Blood tests: Elevated amylase/lipase, WBC, CRP
  • Contrast-enhanced CT scan (CECT): Crucial to assess the extent of necrosis
  • MRI / MRCP: To evaluate biliary involvement
  • Ultrasound / EUS: For gallstones and fluid collections
  • Procalcitonin / Blood cultures: To detect sepsis

Complications of ANP

  • Infected pancreatic necrosis
  • Multi-organ failure (lungs, kidneys, heart)
  • Pancreatic abscess or pseudocyst formation
  • Portal vein thrombosis
  • Persistent systemic inflammatory response (SIRS)
  • Gastrointestinal bleeding
