Mesenteric Ischaemia

Mesenteric Ischaemia

Prompt Diagnosis & Expert Surgical Care for a Life-Threatening Vascular Emergency

What Is Mesenteric Ischaemia?

Mesenteric ischaemia occurs when there is a reduced or blocked blood flow to the small or large intestines, leading to intestinal tissue damage and, if untreated, bowel necrosis (tissue death). It is a serious medical emergency that requires immediate intervention to prevent life-threatening complications.

Types of Mesenteric Ischaemia

  • Acute Mesenteric Ischaemia: Sudden loss of blood flow, often due to a clot or embolism
  • Chronic Mesenteric Ischaemia: Long-term narrowing of mesenteric arteries, usually from atherosclerosis
  • Non-occlusive Mesenteric Ischaemia: Reduced flow without an actual blockage, common in critically ill patients

Causes

  • Blood clots in mesenteric arteries (embolism or thrombosis)
  • Atherosclerosis (narrowed arteries due to plaque buildup)
  • Heart conditions (e.g., atrial fibrillation, heart failure)
  • Hypotension or shock
  • Vasospasm from certain medications or conditions

Symptoms

Acute Mesenteric Ischaemia:

  • Sudden, severe abdominal pain out of proportion to physical findings
  • Nausea, vomiting, or diarrhea
  • Blood in stool
  • Rapid heartbeat, low blood pressure
  • Symptoms of sepsis or shock in advanced stages

Chronic Mesenteric Ischaemia:

  • Recurrent post-meal abdominal pain (intestinal angina)
  • Fear of eating (leading to weight loss)
  • Bloating or diarrhea

Acute mesenteric ischaemia is a surgical emergency. Delays can be fatal.

Diagnosis

Timely diagnosis is essential to prevent irreversible bowel damage:

  • CT Angiography – gold standard for identifying vascular blockages
  • Doppler ultrasound – to assess blood flow
  • Blood tests – may show lactic acidosis or elevated white blood cell count
  • Mesenteric arteriography – for detailed visualization and possible intervention
