Overview

Portal hypertension is a condition where there is an abnormal increase in blood pressure within the portal venous system—a major vein that carries blood from the intestines, stomach, spleen, and pancreas to the liver. It is most commonly caused by liver cirrhosis, but may also occur due to other vascular or hepatic disorders.

What Causes Portal Hypertension?

  • Liver cirrhosis (most common)
  • Chronic hepatitis B or C infections
  • Alcoholic liver disease
  • Non-cirrhotic portal fibrosis (NCPF)
  • Budd-Chiari Syndrome (hepatic vein blockage)
  • Portal vein thrombosis

Symptoms & Signs

While early stages may be silent, advanced portal hypertension leads to:

  • Enlarged veins (varices) in the esophagus or stomach
  • Life-threatening variceal bleeding (vomiting blood or black stools)
  • Splenomegaly (enlarged spleen)
  • Ascites (fluid accumulation in the abdomen)
  • Encephalopathy (confusion due to toxin buildup)
  • Caput medusae (dilated veins on the abdominal wall)

Diagnosis

  • Ultrasound with Doppler – assesses blood flow in the portal vein
  • CT scan / MRI Abdomen – evaluates liver and vascular structures
  • Endoscopy (UGI scopy) – detects varices and bleeding sites
  • Liver function tests
  • Hepatic venous pressure gradient (HVPG) – for accurate pressure measurement
